You are cordially invited to join us in Kuala Lumpur for the ADFM National Caregivers Conference 2016 which
is a joint organised event with
University of Malaya in conjunction with the global commemoration of World Alzheimer’s Month in September each
year.
DATE : September 24 & 25, 2016 (Saturday & Sunday)
THEME: “Caregiver’s Journey”
VENUE: Auditorium, UM Research Management and Innovation Complex, University of Malaya (Main Campus), Jalan Universiti, Petaling Jaya
September month will mark the yearly global campaign to spread greater awareness of
dementia among the public; to change the stigma and people’s perceptions on
Alzheimer’s disease (AD); to reflect on what we can do to support and improve
the lives of people with dementia and their caregivers.
This year our conference theme is “Caregiver’s Journey” and it reflects ADFM’s objective to engage with carers of people suffering from dementia and healthcare professionals. To assist caregivers who wish to attend this conference but unable to leave their charge, we are providing special care service - our trained personnel will engage their loved ones in fun-filled and supervised activity during the 1+1/2 day conference. Please note this service is limited to 30 registered participants (AD persons) and terms and condition applies.
The event is a one and a half days of seminars, talks, and forums, covering an array of topics such as medical and psychological management of Alzheimer’s disease, support and care resources for carers, nutrition, financial and care planning and developments toward person-centred dementia care. Our speakers include specialists from various fields: doctors, psychologists, educators, geriatricians, and professor of law. The forum is an opportunity not to be missed, especially for those who wish to engage with a panel of specialist doctors.
The Conference Program encompasses talks, seminars and forums. Plus for the first time
ever, we offer a unique session which will be conducted in Mandarin. Seats
availability is based on first-come-first-served basis.
Conference fee
is RM50 per person for participants of all categories. Conference Fee is waived for AD persons and their caregivers
who have registered with ADFM Daycare Centre and our respective State Support
Groups (more details, refers to
Registration Form).
Registration is compulsory on
first come first basis. Walk-In will not be accepted due to logistics.
Certificate of Attendance:
Will be issued to healthcare professionals and workers (please provide full name and NRIC No. Participants are to collect their certificate from the registration desk after the caregivers’ forum on 25 September.)

(2) Upon confirmation of registration, make
immediate direct payment via online/ATM to ADFM Bank Account with CIMB Bank at
Bank Account No. 800 229 3277. (If
Cheque, the cheuqe must be crossed and addressed to: “Alzheimer’s Disease
Foundation”.)
(3) Email copy of your Payment Receipt to: jenny@adfm.org.my OR Fax
to: 603 – 7960 8482 for reconfirmation of your registration.
(4) An acknowledgement will be sent to you upon receiving
your Payment Receipt.

A Memory Walk will be held by
ADFM jointly with University Malaya (UM) Medical Faculty 6th College and the
Wellness Cluster Unit. More details will
be announced later.
NO Registration is
required. FREE and open to all the
conference participants, their families and the general public. T-Shirts will be given to participants on the
memory walk day, availability on first come first basis.
By taking
part in the Memory Walk, you are playing an important role in raising greater
public awareness and recognition on the need for practical and emotional
support to people living with dementia and their caregivers.
